Sciatica · Crows Nest · Lower North Shore
Sciatica chiropractor in Crows Nest.
Pain, pins and needles or weakness down the leg is worth getting assessed properly. We work out what's irritating the nerve, settle it, and rebuild the back and hips so it stays settled.
What sciatica actually means
Sciatica is a description of symptoms rather than a diagnosis. It refers to pain that travels from the lower back or buttock down the back or side of the leg, often with pins and needles, numbness, burning or a heavy, weak feeling. It can be constant or come in waves, and it's frequently worse with sitting, bending or first thing in the morning.
The most common cause is irritation or compression of a nerve root in the lower spine, often related to a disc that has bulged or to degenerative narrowing around the nerve. Less commonly the nerve is irritated further along its path through the buttock. Muscle referral from the glutes and hip can also mimic the pattern, which is one reason a proper examination matters.
What we assess
- Red flag screening, including saddle numbness, bladder or bowel change and progressive weakness
- Neurological testing of power, reflexes and sensation to identify the level involved
- Nerve tension and directional preference testing to find positions that reduce your leg symptoms
- Lumbar spine and hip movement, and how you load them through sitting, bending and lifting
- Glute, hamstring and trunk strength
How we treat sciatica
Step one is reducing the irritation. We identify the movements and positions that centralise your symptoms, meaning the leg pain retreats back towards the spine, and use them as your daily program. Hands-on care through the lumbar spine, pelvis and hip musculature reduces protective tone and restores movement, with dry needling where deep gluteal tone is contributing.
Step two is rebuilding. Nerve-related leg pain tends to leave people deconditioned and cautious. Progressive hip and trunk strength, hinge and squat patterning, and a return to walking and training volume rebuild both capacity and confidence. We do that in the gym space at PT Central, with the actual loads you need.
You'll get a clear expected timeline, the specific signs that mean things are improving, and the specific signs that mean we escalate to imaging or medical referral.
Getting back to training and work
Sitting is often the hardest part, especially for desk-based work along the Pacific Highway. We give you practical modifications for the first few weeks, then reintroduce sitting tolerance, deadlifting, running and everything else in a staged way instead of leaving it to chance.
Frequently asked questions
Is sciatica a diagnosis?
Not really. Sciatica describes a pattern of symptoms: pain, pins and needles or weakness travelling down the leg along the path of the sciatic nerve. The job is to work out what's irritating the nerve and what reduces that irritation for you.
Do I need surgery?
Most people don't. The majority of nerve-related leg pain improves with conservative care over weeks to a few months. Surgical opinion is reserved for progressive weakness, unmanageable pain, or symptoms that fail to improve with good conservative management.
How long does sciatica take to settle?
Leg symptoms usually take longer than plain back pain. Many people notice the pain drawing back towards the spine over a few weeks, which is a good sign, with full recovery over one to three months depending on severity.
Are there symptoms I should not wait on?
Yes. Progressive weakness in the leg or foot, numbness around the groin or saddle area, or loss of bladder or bowel control need same-day medical assessment. We screen for these at your first visit and will refer immediately if present.
Can a chiropractor help sciatica?
Chiropractic care can help by improving movement in the lower back and hips, reducing protective muscle tone, and guiding the loading and positions that reduce nerve irritation, alongside a graded rehab program.
