Hamstring Strains: A Return-to-Play Timeline You Can Actually Trust

Most of the hamstrings I see in Crows Nest didn't fail once. They failed a second or third time, usually a few weeks after the first one felt fine. The decision to go back was based on a date on the calendar, not a test on the day. That pattern is common enough in weekend sport to be worth spelling out properly. The return-to-play question matters more than the injury itself.

Here is what actually happens when a hamstring goes, why it comes back so often, what the evidence on prevention really shows, and how to make the return-to-play call on something more solid than "it's been three weeks."

What actually happens when a hamstring goes

A hamstring strain is a tear in the muscle-tendon unit at the back of the thigh. Most often in biceps femoris, the outer of the three hamstring muscles. It typically happens during high-speed running, at the point in the stride where the leg is decelerating just before the foot lands. At that moment the hamstring is lengthening and contracting hard at the same time. That combination is what tears muscle fibres.

Strains are usually graded from one to three. A grade one is a small number of fibres affected, often just tightness and a pull, with a return to sport inside one to two weeks. A grade two involves partial tearing, more pain, some loss of strength, and typically several weeks out. A grade three is a substantial or complete tear, usually with bruising and a visible gap. That one takes much longer, sometimes with a scan and a specialist opinion involved. None of these grades is a guess. A proper assessment can tell fairly quickly which category an injury sits in. That should happen first, not last.

Location matters too, not just severity. A strain low down near the sit bone, at the tendon's attachment, tends to behave differently to one in the middle of the muscle belly, and often takes longer to settle. That is one of several reasons two people with what feels like "the same" injury can end up on quite different timelines, and why a timeline copied from a teammate is not a plan.

Why it comes back

The single strongest predictor of a hamstring strain is a previous hamstring strain. Reinjury inside the first year is common. It happens more often when the return to sport is judged by time rather than by how the muscle is actually performing. A few other factors raise the risk too: a low ratio of hamstring strength to quadriceps strength, reduced knee extension flexibility, and simply running at high speed without having built up the capacity to tolerate it. Hamstring force rises sharply as sprint speed increases toward maximum. That is exactly why strains cluster around sprinting rather than jogging (Erickson & Sherry, 2023, "Prevention and Rehabilitation of the Athletic Hamstring Injury," PMC).

None of that is really bad luck. It is a muscle going back to full sprinting load before it can tolerate full sprinting load. That is a training and rehab problem, more than an injury problem.

Nordic curls: the honest version

The Nordic hamstring curl has become close to standard advice for hamstring injury prevention. You kneel, then lower your body forward under control while a partner or anchor holds your ankles. Several pooled analyses of prevention programs reported that adding the exercise roughly halved hamstring injury rates in team sport.

That figure is weaker than it sounds. A 2021 methodological reappraisal went back through the trials feeding those pooled analyses. Once inconsistent injury definitions and other methodological issues were accounted for, the injury-preventive effect became inconclusive (Impellizzeri et al., 2021, Journal of Clinical Epidemiology). That does not mean the exercise does nothing. It means the confident "cuts your risk in half" framing overstates what the underlying evidence can actually support.

The sensible position sits in the middle. Eccentric hamstring strength is a plausible protective factor. The exercise is low cost and low risk. There is no good reason to leave it out of an off-season or in-season program. There is good reason to stop presenting it as a guarantee.

What return to play should actually be based on

This is the part most return-to-play advice skips. It is also the part that actually predicts whether the injury comes back. Two broad approaches show up in the research.

The first relies mainly on strength testing, using equipment such as an isokinetic dynamometer to check the injured hamstring has recovered strength close to the uninjured side. Athletes cleared this way tend to return faster, often within twelve to twenty-five days. Reinjury rates in the following months sit higher though, in the range of six to fourteen percent.

The second is built around functional and sport-specific testing, such as the Askling H-test, a controlled maximal-speed hamstring stretch and strength test. It takes longer, often thirty-six to sixty-three days. Reinjury rates drop substantially though, down to roughly one to four percent (Erickson & Sherry, 2023, PMC).

Read plainly, that is a genuine trade-off between speed and safety. Not a case of one method being right and the other wrong. What it rules out is the third, most common approach: no formal testing at all, just a rough sense it feels okay and the season is close. That is the version most likely to end in a second injury, the same pattern that shows up after a rushed return from a rolled ankle.

What this looks like in the clinic

The most common thing I hear from someone who has done their hamstring is not really a question about whether it will get better. It is the question they are avoiding: when can I trust it again. Most expect the answer to be "stop playing," so they never ask it out loud.

That is not how this needs to go. Early management protects the area without going to full rest. Some pain-free movement and light loading in the first days tends to support recovery better than doing nothing, alongside the usual short-term basics of reduced load, ice and compression while things settle. From there, a progressive strength program builds load back up in stages, tracking strength and control rather than the number of days that have passed. Nordic curls, or a similar eccentric exercise, usually feature in that program, alongside exercises that gradually reintroduce the lengthened, loaded position that tends to be where strains happen in the first place. Sprint mechanics work comes in as the return gets closer, since jogging pain-free and sprinting pain-free are two different things, and only one of them is what actually gets tested on game day.

The return-to-play decision itself is based on how the leg performs against the uninjured side, not on a fixed number of weeks. Some presentations clear faster, some need longer. The honest answer to "when can I play again" really is "it depends what the leg can do." Not a date picked in advance to sound reassuring.

The short version

  • Hamstring strains are graded, and the grade should be established early rather than guessed at.
  • The strongest risk factor for a strain is a previous one, and most of that risk comes from returning to full speed before the muscle can tolerate it.
  • Nordic curls are a reasonable, low-risk addition to a training program, but the evidence behind them is more contested than the 'halves your risk' version usually presented.
  • Return-to-play decisions built around real strength and functional testing take longer than a calendar-based return. They meaningfully cut the chance of doing it again.
